INTRODUCTION Fever and intestinal symptoms (mostly diarrhoea) are two leading complaints in ill travellers returning from a journey to tropical or subtropical destinations (1-3). When an international traveller presents with both symptoms concomitantly, one should consider the diagnosis of febrile traveller's diarrhoea (TD), but also of a systemic infection associated with intestinal symptoms (e.g. malaria, typhoid fever or acute schistosomiasis), and even of a systemic infection concomitant with traveller's diarrhoea (dual infection). Therefore, the initial workup of such cases should look simultaneously for common systemic and enteric travel-related infections.
